APPLICATION PROCESS
Families that wish to apply for assistance from the Foundation are asked to submit a letter to the Foundation. The contact to the Foundation comes directly from the family in need of assistance and all correspondence will be between the family itself and the Board of the Foundation.
A letter stating the child’s name, age, address and diagnosis is to be sent directly to the Foundation. Included in the initial contact letter should be parent’s contact and information, who referred them to the Foundation, pertinent medical information, treatment plans if any and how the funds applied for would be used.
The Board of Directors reviews all letters received to determine initial eligibility for assistance.

In order to be eligible for assistance, the following initial criteria must be met:

the child must be 17 years of age or younger.

the child must be resident of South Western Ontario.

the child must suffer from a life threatening illness.

the family must demonstrate that the child’s illness has placed them in financial hardship.
If the Board determines that the family meets the initial eligibility for assistance, the Foundation sends a more detailed application to the family for completion, which permits the Foundation to review in more detail the family's situation and authorizes the Foundation to speak with their medical care providers and make financial inquiries as necessary.
